Submit Button Help

My Contributions
I'm new to programming and I can't seem to find out how to get my submit button to work correctly. I want to have all the info someone fills out sent to an email account. I used Microsoft Publisher and the site is published but the only issue I'm having is the Submit button. Any advice? The website is http://myjohnscreekdentist.com/index_files/Page1426.htm. Thanks!!!

What scripting language are you using to to implement the email functionality? Can you post the code for that functionality? Not the whole page, just the scripting for the email send...

I'm using Microsoft Publisher and I believe its PERL script.
To be honest, I'm not sure if this is what your asking for. THanks for replying !!
