Using a combo box to open a form

I created a form that will allow users to input data. I would like to include a drop down box that, depending on their selection, will open a form where they can enter data as well. Does anyone know how i can do this?

Well for starters what language are you working in so I can move this to the proper forum.

Second:

Dream.In.Code has a policy by which we prefer to see a good faith effort on your part before providing source code for homework assignments. Please post the code you have written in an effort to resolve the problem, and our members would be happy to provide some guidance. Be sure to include a description of any errors you are encountering as well.

I don't know a thing about Access. I have to build a database for my job and i am using Visual Basic for the code. I do not have a code for this yet becuase i do not know how to create one.

VB6 or a different version? I ask this because if it's 2003, 2005 or 2008 I need to move this to the VB.Net forum, otherwise I move it to the Visual Basic forum.
