Url link in php/MySql record brings up wrong url

Here is the php/MySql record. The problem url is at the bottom.

When clicking on Next, I get the following url in the address bar which of course gives me an error. I can't get the full url to show here so you will have to either click on this url to see the address bar or look down at the bottom of your screen to see the problem. (I tried quot tags for this post but the quote tags did not get the full url to show within the post on the forum.)

Thank you for the response, psycho coder.

I have solved this issue. The problem was that the single and double quotes I was using were not straight but were curly. Unless the user blows up a font to perhaps 16 or 18 pt. the differences are barely noticeable. For future refernce if anyone should ever have a similar issue, here is a quote from the Word help file. Presented here for educational purposes only. Fair use applies.

QUOTE
Change curly quotes to straight quotes and vice versa
Microsoft Word automatically changes straight quotation marks ( ' or " ) to curly (smart or typographer's) quotes ( or ) as you type.

To turn this feature on or off:

On the Tools menu, click AutoCorrect Options, and then click the AutoFormat As You Type tab.
Under Replace as you type, select or clear the "Straight quotes" with "smart quotes" check box.
Note You can find and replace all instances of single or double curly quotes with straight quotes in your document. To do this, clear the "Straight quotes" with "smart quotes" check box on the AutoFormat As You Type tab. On the Edit menu, click Replace. In both the Find what and Replace with boxes, type ' or ", and then click Find Next or Replace All.

To replace all straight quotes with curly quotes, select the "Straight quotes" with "smart quotes" check box, and repeat the find and replace procedure.
