How do you insert an audio recorder into your website for users..?

I have a project that aim to allow people who participate in the charity to record their experience in helping children in various poor countries. In order to make it more convenient, I am hoping to put this audio recorder that will enable users to record --> stored in my data base --> show to public in a forum link structure! thanks :)
