Hi guys. I have spent a few days now trying to recreate the following image Here

The front will be exactly the same as seen in the image, the back will be like a contact form where the user can enter their name and number.

I was hoping someone could take a look at what I have done and advise me on how to get it exactly the same. I have partly got what I want, but I just cant achieve the exact same look, especially in regards to the tabs.

that looks like it is 2 images-- one behind the other to create the illusion of a tabbed page....I can't click on files at work or I would tell you for sure
