Using Crow’s Foot notation, create ERD(VISIO/MySQL) for medical clinic.
1. A patient can make many appointments with one or more doctors in the clinic, and a doctor can accept appointments with many patients. However, each appointment is made with only one doctor and one patient and the date and time of the appointment is recorded. No appointment is required in emergency cases and such appointments are recorded as “unscheduled”.
2.If an appointment is kept, it yields a visit with the doctor specified in the appointment. A visit results in a diagnosis.
3.Each patient visit generates a bill by the doctor.
4.The clinic sends the bill to the patient’s insurance company and to the patient. (Typically, a portion of the total bill is paid by the patient’s insurance company and the rest by the patient.) Assuming that a patient may have at most one insurance company.
5.The patient may pay the bill in full or in many installments. A payment may cover more than one bill.
