ListBox Need to add quantity for each item selected

Need a way to add quantity to each selected item on listbox. Attached, if it works, is the db in 2 parts for size constraints.

Attached is first part of db.It did not let me attache both db's at a time. The pieces of both db's need to be put together into one.
