Up load images and video to web site

Hi,

I hope this question is within the scope of this site.
I'm looking for a script that lets browsers fill out a form and upload images and video files from their devices to a file on my website. Then I can down load them for editing.

Any advice, direction or developer will be helpful.

Are you looking to hire a developer to do this, or are you looking to make this yourself?
