Make dymanic text boxes, give them varible names, submit them to a php/cgi/asp script that mails those varibles.
I suggest looking to flash kit for a tutorial.

flash mx, i believe has a feature that will take care of all that stuff for you....

Everything since alteast 4 has been able to do that, but MX doesn't have a automatic form generator or anything. You still need to do it by building the boxes and sending it to a script.

the boxes are automatically built for you already actually....i was playing with it the other day....
