creating flash form to generate txt file

I have a txt file with variables that are loaded into my movie. I need a form in flash or html that will generate the end file into a txt file.

Below is the contents of my text file, and underlined are my variables.

textDate=Date: 6/22/2008&textName=Name: Dr. Goss&textTime= Time: 8am - 8pm&textRoom=Room #: 1400

Could someone please assist me with this time-sensitive matter.

idk how to do it in HTML or Flash, but I know you can use PHP to do it... >.> probably not too helpful, I know....
