create dialog box

can somebody give me example how to create a dialog box in c++. i just have to enter some data like first name ,last name, address , zipcode and phone number though dialog boxes . it is just a little part of my program that i can't figured out. i tried to look for info on this topic everywhere but it is too confusing.
