How can we print a report with header and footer using PHP?

How can we print a report of 20 records student's name with header and footer using php? I mean using mysql query, loop and page break.
Let say the total of students record is 100.
The report format should display 20 records of student's name per page and have Header and Footer with a fixed position.

Are you just outputting regular text?

yes, its only regular text. It's simple. The problem is When student's name that have many character this will drag the footer below to the next page. I want the footer stick (fixed) position not move anywhere. do u have any idea to do that?
