    Setup a script to get the information and validade it, you can find a lot of them with a simple Google search. You can either use AJAX to submit the information to the script and check if the it's valid then submit the form, or use the awesome HTML5 Forms.

    I would suggest you to take a look at CSS Transitions, a great article to understand them is Using CSS transitions. Another great resource for your case would be Cross fading images with CSS Transitions.

    Using databases isn't an easy thing. The best way to do what you want is just create a simple PHP script (if your server supports PHP) to receive the form's content and check if they are equal to the ones you want. If you want to learn more about how to get the form information via PHP I suggest you to read PHP: Forms and User Input.
