can cloud databases be accessed by an unauthorised user

Presuming you had your data backed up in the cloud database;can it be in risks of being hacked.

No more risky than having it hosted anywhere else.

Technically, yes, just like enough supercomputers thumping away at GUID collisions could find one before the universe ends. But for practical purposes, an Azure SQL database is probably more secure than an on-prem SQL Server instance on a Windows Server 2012 VM on the domain of a company of average technological maturity. Microsoft has more dedicated security personnel for Azure than 99% of companies have in their IT departments. I know the question isn't Azure specific, but that's my area of expertise, and it's no different for any other mature cloud service provider.
